Deductible: $250 Individual / $500 Family  

• Individual: $1,000,000
• Family: $5,000,000

• Primary Care Office Visits: $0 after deductible (10 visits max)
• Specialist Office Visits: $50 after deductible (10 visits max combined with PCP and Urgent Care)
• Urgent Care: $50 after deductible (10 visits max combined with PCP and Specialist)
• Telemedicine Services: $0 copay for virtual primary care (12 visits max), urgent care, and mental health (4 visits max)

• Emergency Room Care: $250 copay after deductible (limit: 2 accident-related visits and 2 sickness-related visits per year)
• Emergency Medical Transportation: Covered after deductible (limit: 2 visits, ground and air combined)

•  Outpatient Hospital / Surgical Center: 100% after deductible (limited to 3 surgeries per plan year)
• Inpatient Hospital Services: $1,000 copay per admission after deductible (limited to 2 non-ICU hospital stays and 3 ICU stays per year, with a 10-day stay max)

• Preventive Screenings (e.g., Pap Smears, Mammograms, Immunizations): 100% covered (subject to limitations based on ACA requirements) 

•  Telemedicine Mental Health Visits: $0 copay (4 visits max)
• Inpatient/Outpatient Mental Health Services: Not covered except for telemedicine services

• Home Health Care: Covered up to $500 per year
• Skilled Nursing Care and Hospice Services: Covered up to $5,000 per year
• Therapies (e.g., PT, OT, ST, Chiropractic): 10 visits per year, $50 copay after deductible
• Durable Medical Equipment: Covered up to $500 per year
• Chemotherapy/Radiation: Covered up to $50,000 per year
• Prescription Drugs: No copay for generic drugs through MyLivePharmacy's formulary
